Price rises, a state you moved to, a dose nobody will adjust, or a platform you simply cannot reach. Switching is common and mostly straightforward, but the sequencing matters. Here is how to move providers without running out mid-transition, and what to get in writing before you cancel anything.
Switching online HRT (partner link: we may earn a commission, at no extra cost to you) providers is more common than the platforms would like, and there is nothing dramatic about it. The one thing that goes wrong is sequencing: women cancel the old subscription first, then discover the new provider needs a week, and end up with an unplanned gap.
Here is the order that avoids that, and what to collect before you move.

Write down exactly what you are currently taking. Not "the patch" or "my cream". You want the specific hormone, the dose, the form, and the schedule. For example: estradiol patch 0.05 mg twice weekly, plus micronized progesterone 100 mg nightly.
This matters more than people expect. A new clinician who knows precisely what you are on can usually continue it. A new clinician working from "I think it was the middle strength" is starting over, and you may spend months re-titrating a dose you had already got right.
Most platforms show your prescription details in your account. If yours does not, message them and ask for it in writing before you cancel, because access often disappears with the subscription.
Also note: whether your current preparation is compounded or FDA-approved, whether you have had any side effects and at which dose, and any dose changes you have already been through. All of it shortens the new intake.
Overlapping by a few weeks costs you one month of the old plan. That is the price of not running out, and it is usually worth paying. If money is tight, time the switch for just after a delivery arrives so you have a full cycle in hand. Our guide to how online refill timing works explains what to do if a refill runs late.

You are six weeks in and do not feel better yet. That is often too early. Hot flashes typically improve over two to six weeks, mood and joint symptoms take longer, and vaginal symptoms can take eight to twelve. Switching platforms restarts the clock without changing the medicine.
You are having side effects. Message your current clinician first. Many side effects can be managed by changing the dose or the route, which is a conversation, not a reason to change companies. If they will not have that conversation, then switch, and now you have a real reason. The exception, if you take estrogen: sudden changes in vision or speech, a sudden new severe headache, or severe chest pain or shortness of breath need emergency care. Call 911 rather than waiting for a reply. Severe leg pain, or another unusual symptom that worries you, also needs prompt medical attention.
Expect a full intake even though you are already established on treatment. They are prescribing, so they carry the responsibility, and they cannot rely on someone else's assessment.
Be straightforward that you are transferring rather than starting fresh, and give them your current regimen. Most clinicians will continue an existing dose that is working rather than restart you at the bottom, but only if they know what it is.
Cancel carefully. Do it in writing where you can, keep the confirmation, and check your card in the following month rather than assuming.
Watch for auto-renewal timed to a cycle rather than a calendar month, cancellation windows that require several days' notice, and "pause" options presented in place of cancellation. Our guide on cancelling an online HRT subscription covers the specifics.
Since you are switching partly on service, weigh what is actually included: whether clinician messaging is bundled or charged, whether dose adjustments are included, whether the price covers a complete regimen or a single product, and which states are served.
Our ranked comparison of online HRT providers covers those directly, and the 60-second match quiz will shortlist based on what pushed you to switch.
