How Online HRT Refills Work (and What to Do If You Run Out)

Most online HRT runs on a subscription, so refills happen automatically until they do not. The gaps tend to appear for boring reasons: an expired card, an annual reassessment nobody flagged, a shipping delay, or a prescription that quietly ran out of authorised refills.

Here is how the system actually works and what to do when it does not.

How automatic refills work

When you subscribe, the platform sets a recurring cycle, commonly monthly or quarterly. A few days before each cycle it charges your card and triggers the pharmacy to dispense. The medication ships, and the loop repeats.

Two things sit behind that loop that are easy to forget. Your prescription has an expiry date and a finite number of authorised refills, and most platforms require a periodic clinical reassessment, often annually, before renewing it. That reassessment is usually a short questionnaire rather than anything onerous, but if you ignore the email, the refill stops.

Quarterly cycles are cheaper per month on some platforms and worth asking about, with the trade-off that you commit further ahead.

The five common reasons a refill fails

• An expired or declined card. The single most common cause, and the easiest to fix. Update it before you travel and before a card reissue date.
• An overdue reassessment. Check your spam folder. These emails are routine and easy to miss, and the refill will not release until it is done.
• A prescription that reached its refill limit. Needs the clinician to reauthorise, usually one to three days.
• A pharmacy or supply delay. Compounded preparations are made to order, so they are more exposed to this than manufactured products pulled from stock.

What actually happens if you miss doses

Worth knowing so you can judge how urgently to act.

Missing estrogen generally brings symptoms back rather than causing harm. Hot flashes and night sweats often return within days, and sleep usually goes with them. It is unpleasant rather than dangerous, and it resolves when you restart.

Missing progesterone while continuing estrogen is the one that matters. If you have a uterus, progesterone is what protects the uterine lining from unopposed estrogen. A few missed days is not an emergency, but do not settle into taking estrogen alone. If you are going to run short of one, run short of both rather than continuing estrogen by itself, and tell your clinician.

Missing vaginal estrogen is slow-acting in both directions. A week off changes little; a few months off and the tissue reverts.

Practical fixes when you are about to run out

1. Message the provider and say plainly that you will run out on a specific date. Vague messages get routine handling; a date gets prioritised. Most platforms can expedite.
2. Check whether the hold is billing or clinical. Billing you can fix yourself in two minutes. Clinical needs the clinician.
3. Ask for a bridge supply if there is a genuine delay. Many platforms will send a short supply to cover a gap.
4. If you are on an FDA-approved product, ask about a local pharmacy fill. Approved estradiol and micronized progesterone are stocked at retail pharmacies, so a prescription can sometimes be sent locally to cover a gap. This is not possible with compounded preparations, which is a practical advantage of approved products that rarely gets mentioned.
5. Do not ration by cutting patches or splitting capsules without asking. Some forms tolerate it and some do not, and micronized progesterone capsules in particular are not designed for it.

Build a small buffer

The simplest protection is to stay slightly ahead. Request your reassessment early rather than on the deadline, keep your card current, and if your platform allows it, shift your cycle so deliveries land a week before you would actually run out.

A one week buffer absorbs almost every delay that occurs in practice. Two weeks covers nearly all of them.

What good looks like

If refill problems keep happening, that is a service issue rather than bad luck, and it is a legitimate reason to move. What you want is a provider where you can reach a clinician without paying per message, where reassessment prompts arrive with notice, and where a delay produces a bridge supply rather than a shrug.
