A personal history of blood clots does not automatically rule out hormone therapy. Here is what the research really shows about transdermal estrogen (the patch, gel, or spray) for women with a clotting history, why the route of delivery changes the risk, and how to have an informed conversation with the right provider.
If you have ever had a blood clot, whether a deep vein thrombosis in your leg, a clot that traveled to your lungs, or a stroke, you have probably been told at some point that hormones are off the table. Maybe a doctor said it quickly, maybe it was written on a chart years ago, and maybe no one ever revisited it. So now you are deep in perimenopause or menopause, the hot flashes are relentless, the sleep is gone, your joints ache, and you feel like the one menopause treatment that actually works was locked away from you. That is a genuinely hard place to be, and you are not imagining the unfairness of it.
Here is the part that does not always get explained clearly: a history of blood clots does not automatically close the door on every form of hormone therapy. The conversation has changed a great deal, and the way estrogen is delivered into your body turns out to matter enormously. This article walks through what the research actually shows about transdermal HRT (estrogen absorbed through the skin) when you have a clotting history, why the route of delivery changes the risk picture, and how to have a thoughtful, fully informed conversation with a provider who understands the nuance.

For decades, the warnings about hormone therapy and clots were built almost entirely around oral estrogen, the kind you swallow as a pill. And that warning was earned. When you take estrogen by mouth, it travels straight from your gut to your liver before it ever reaches the rest of your body. This is called first-pass metabolism, and the liver is where many of your clotting factors are made.
That first pass through the liver nudges the body toward clotting. Oral estrogen increases the production of certain clotting proteins, raises thrombin generation (thrombin is a key player in forming a clot), and can create a state of resistance to a natural anticoagulant called activated protein C. In plain terms, swallowing estrogen tilts your internal chemistry slightly toward forming clots. For most healthy women that shift is small and clinically unimportant, but for a woman who has already had a clot, that small push lands on top of an existing vulnerability.
This is why so much of the older fear around HRT, especially the messaging that followed the Women's Health Initiative in the early 2000s, centered on clots and strokes. But that landmark study used a specific oral estrogen (conjugated equine estrogens) taken by mouth. It did not test estrogen delivered through the skin. For a long time, the warning got applied to all hormone therapy as if the delivery method did not matter. It turns out it matters a lot.
Transdermal estrogen, delivered through a patch, gel, or spray, is absorbed directly through your skin into your bloodstream. It bypasses that first concentrated pass through the liver. Because it never floods the liver the way a swallowed pill does, it has very little effect on the clotting proteins the liver produces.
The evidence here is not a hunch. A systematic review published through researchers affiliated with the Mayo Clinic, pooling roughly fifteen observational studies, found that oral estrogen was associated with a meaningfully higher risk of venous thromboembolism, with a relative risk on the order of 1.6 compared with transdermal estrogen, and an even larger relative difference for deep vein thrombosis specifically. Crucially, in study after study, transdermal estrogen has shown a risk of venous clots that is essentially the same as taking nothing at all. One earlier pooled analysis put the venous clot risk ratio for transdermal users at right around 1.0, meaning no detectable increase.
The reason is mechanical and biological, not magical. Laboratory studies looking at coagulation markers consistently show that transdermal estradiol has minimal effect on the hemostatic system, while oral estrogen measurably shifts it. So when a thoughtful menopause specialist tells you the patch is different from the pill, they are standing on solid, reproducible science. This is where many women are pleasantly surprised. Major bodies have moved toward recommending the transdermal route specifically for women at higher risk of clots, rather than ruling out hormones entirely.
The United Kingdom's National Institute for Health and Care Excellence (NICE), in guidance updated in 2024, explicitly recommends considering transdermal rather than oral hormone therapy for women at increased risk of venous thromboembolism, including those with a BMI over 30, while advising specialist haematology referral before starting HRT for women at high risk such as those with a strong family history of clots or a hereditary thrombophilia. The Menopause Society (the leading North American professional body, formerly the North American Menopause Society) similarly recognizes the transdermal route as the preferred option when thrombotic or cardiovascular risk is a concern. The Endocrine Society and ACOG (the American College of Obstetricians and Gynecologists) likewise point to transdermal delivery as the lower-risk choice for women with vascular concerns, and these bodies emphasize using the lowest effective transdermal dose, with patches commonly started at 0.025 to 0.05 mg per day and titrated to symptom control.
Real-world data continue to reinforce this. A 2025 case series followed more than one hundred women who had a personal history of venous or arterial thromboembolism and who used transdermal estradiol HRT for over a year. Across that group, transdermal estrogen did not appear to drive an excess of recurrent clots. That does not make it a guarantee of safety for everyone, and it is not a randomized trial, but it adds to a consistent body of evidence that for many women with a clot history, transdermal HRT can be considered rather than reflexively refused.

If you still have your uterus, you will need a progestogen alongside estrogen to protect the uterine lining. And here is a detail that often gets overlooked: not all progestogens behave the same way when it comes to clotting.
The French ESTHER study, one of the most cited investigations on this exact question, found that micronized progesterone (the body-identical form, often sold as Prometrium) did not appear to add to clot risk, while certain older synthetic progestins, particularly the norpregnane derivatives, were linked to a several-fold increase in venous clots. This is why menopause specialists treating a woman with a clotting history will often pair transdermal estradiol specifically with micronized progesterone, or sometimes deliver progestogen through a hormonal IUD, to keep the whole regimen as clot-neutral as possible.
So the safest-leaning combination, in the eyes of much of the current literature, tends to be transdermal estradiol plus micronized progesterone, kept at the lowest effective dose. It would be dishonest to suggest transdermal HRT is automatically fine for every woman who has had a clot. The honest answer is that it depends on the details of your history, and those details deserve real attention.
A provider will want to know things like: Was your clot provoked by a clear temporary cause (such as surgery, a long flight, a leg cast, or estrogen-containing birth control) or did it happen out of the blue with no obvious trigger? An unprovoked clot generally signals a higher ongoing tendency than a provoked one. Have you ever been tested for an inherited clotting disorder such as Factor V Leiden? Are you currently on a blood thinner? Have you had an arterial event like a stroke, which raises somewhat different considerations than a vein clot? Your weight, blood pressure, smoking status, and migraine history all feed into the picture as well. Many providers will also coordinate with the hematologist who managed your clot, or refer you to one, so that two specialists are weighing in together. That kind of collaborative care is a feature, not a delay. It means your treatment is being built around your actual body rather than a one-size-fits-all warning.
If a general practitioner declined to even discuss hormones the moment they saw your clot history, please know that this is common and that it does not have to be the end of the conversation. Many clinicians who do not specialize in menopause are understandably cautious and apply the older blanket warnings. A provider who treats menopause regularly is far more likely to be comfortable with the route-of-delivery nuance and the current guidelines.

The clearest message from the current evidence, echoed by NICE, The Menopause Society, the Endocrine Society, and ACOG, is that the route by which estrogen enters your body changes the clotting picture profoundly, and that transdermal estrogen, often paired with micronized progesterone, is the option most likely to be on the table for women like you.
