Every menopause telehealth site promises speed. Few of them tell you where the days actually go, which matters if you are trying to decide whether to wait four months for a specialist appointment or start online next week.

Day 0: intake. Fifteen to thirty minutes of questions about your symptoms, menstrual history, medical and family history, medications, and whether you still have a uterus. Longer than the "two minute quiz" in the advertising, and that is a good sign rather than a bad one.
Day 0 to 2: clinician review. A licensed clinician reviews your submission. Three things can happen: they prescribe, they come back with follow-up questions, or they decline and explain why. Most platforms turn this around within a business day, though weekends and holidays are real.
Day 1 to 3: pharmacy. The prescription goes to a pharmacy for dispensing. Compounded preparations are made to order, which can add a day or two compared with a commercially manufactured product pulled from stock.
Day 2 to 7: shipping. Most platforms ship in one to five days. Some advertise one to two day delivery.
Realistic total: about three to seven days from finishing the intake to having the medication in your hand. Under three days is possible but not typical. Two weeks means something specific went wrong, usually one of the items below.

The honest comparison is not days versus days. It is days versus months.
Getting a new appointment with a menopause-certified specialist commonly means a wait of several weeks to several months. A primary care appointment is faster, but carries a meaningful chance of being refused or redirected, which is a problem we cover in what to do when your doctor will not prescribe HRT.
Against that, the in-person route gives you an examination, an established relationship, and someone who can order and interpret whatever else is going on. Neither route is simply better. But if speed is your deciding factor, the gap is large and it is not close.
1. Rushing the intake. It is tempting to skim, particularly the family history section. Every omission that triggers a follow-up costs more time than filling it in properly would have. Set aside twenty uninterrupted minutes and do it once.
2. Not checking state coverage first. Check before you spend twenty minutes on an intake, not after. This is the most common cause of starting over with a different provider. Getting the medication quickly is not the same as feeling better quickly, and it is worth separating the two so you do not conclude too early that treatment has failed.
Hot flashes and night sweats often improve within two to six weeks. Sleep frequently improves early. Mood and joint symptoms tend to take longer. Vaginal symptoms are the slowest, often eight to twelve weeks for meaningful tissue change. Dose adjustment during the first few months is normal and expected, not a sign something has gone wrong.
