One is the estrogen your own ovaries made. The other is a mixture derived from horses, and it is what most of the scary research was actually done on.
Two prescriptions, both called estrogen, and nobody explains why you were handed one rather than the other. If you have been on Premarin for years, or you have just been offered it, the difference is worth understanding, because it is not a matter of brand preference. These are chemically different medicines with different evidence behind them.
The short answer: estradiol is the same molecule your ovaries produced, and it comes as a patch, gel, spray or tablet. Premarin is conjugated equine estrogens, a mixture of estrogens originally derived from the urine of pregnant mares, and it only comes as a tablet or a vaginal cream. Most menopause specialists now start with transdermal estradiol, because skin absorption avoids the liver pass that carries the clot risk, and because generic estradiol is far cheaper. Premarin is not a bad medicine, but it has no transdermal form, which is the main reason it has fallen out of first place.

Estradiol, more precisely 17 beta estradiol, is the main estrogen your ovaries made before menopause. What you are prescribed is structurally identical to it, which is what the word "bioidentical" properly means. That term has been muddied by marketing around custom compounded preparations, but a standard estradiol patch from a pharmacy is bioidentical in the only sense that matters chemically. Our page on what bioidentical actually means untangles that confusion.
Because estradiol is available as a patch, gel and spray as well as a tablet, you can choose how it enters your body, and that choice turns out to matter more than the molecule itself for several important risks.
Premarin is a mixture. It contains estrone sulfate along with several equine estrogens that the human body does not make, notably equilin and equilenin. The name is a contraction of "pregnant mares' urine", which is where it is sourced. It has been in use since the 1940s, which means it has an unusually long track record, and it is the estrogen that the largest and most consequential trials were built on.
Almost everything the public believes about hormone therapy risk traces back to the Women's Health Initiative, which reported in 2002. That trial used conjugated equine estrogens, and in the combined arm it paired them with medroxyprogesterone acetate, a synthetic progestin.
This is not a footnote. It means the risk figures that shaped two decades of practice were generated using one specific oral formulation, in a study population whose average age at enrolment was 63, many years past menopause. Applying those numbers unchanged to a 50 year old starting a transdermal estradiol patch is a category error, and it is one that the field has spent twenty years correcting. Since February 2026 the FDA has been approving updated labels, product by product, that narrow the warning, and we covered what changed and what it means separately.
This is the single biggest difference between the two, and it is not really about the molecule. Any estrogen you swallow goes to the liver first in concentrated form, and the liver responds by producing more clotting factors. Estrogen absorbed through skin does not do this to anything like the same degree.
Premarin exists only as an oral tablet, or as a vaginal cream for local use. So choosing Premarin for systemic treatment means choosing the oral route by default. Estradiol lets you choose. If you have any clot history, migraine with aura, raised blood pressure or a higher body weight, that choice is likely to matter to your clinician. Our patch versus pill comparison goes into it properly.
Generic estradiol, in patch or tablet form, is one of the cheaper prescriptions in the pharmacy. Premarin has no generic equivalent, because a mixture of equine estrogens cannot be replicated the way a single molecule can, and it is typically considerably more expensive. If cost is a factor, this is a real one. Our guide to getting HRT without insurance covers discount options.
You cannot map doses across directly, and this catches people out when switching. The two are not interchangeable milligram for milligram, so a switch should be prescribed rather than improvised.
Expect a settling period of a few weeks after any change, and do not judge the new arrangement in the first fortnight. Symptoms often wobble before they improve, which is true of most hormone adjustments and is not a sign the new prescription is wrong. Our piece on how long HRT takes to work covers realistic timescales for each symptom.
Both are available as a local vaginal treatment, and at those doses the systemic considerations largely fall away, because very little is absorbed into the bloodstream. If your issue is dryness or discomfort rather than hot flushes, that is a different decision entirely. Our page on vaginal estrogen forms compares creams, tablets and rings.

Any honest comparison has to mention this, because focusing only on the estrogen misses half of what that trial actually tested.
In the combined arm, conjugated equine estrogens were paired with medroxyprogesterone acetate, a synthetic progestin. The breast cancer signal that dominated the headlines came from that combined arm. The estrogen-only arm, in women who had had a hysterectomy and therefore did not need a progestogen, did not show the same increase, and over long-term follow-up it pointed in the other direction.
That is a substantial detail to have lost in the public conversation. It suggests the progestogen component carried meaningful weight in the finding, and it is one of the reasons micronised progesterone, which is structurally identical to the progesterone your body made, is now widely preferred over older synthetic progestins where it suits the woman.
So "estradiol versus Premarin" is not the only axis that matters. If you have a uterus, what you are paired with is arguably the more consequential half of the prescription. Our progesterone page covers the types, and the oral versus vaginal comparison covers the routes.
If you are on Premarin plus a synthetic progestin and you have been anxious about the risk figures you have read, it is worth knowing that the specific combination you are on is the one those figures came from, and that alternatives exist. That is a reasonable thing to raise at a review. It is not a reason to stop anything abruptly.
Yes, and it would be dishonest to pretend otherwise. Some women have been stable and well on it for years, and there is rarely a good reason to disrupt something that is working. It has the longest safety record of any hormone therapy product in the United States, simply by virtue of having been available since the 1940s. And a small number of women report doing better on it than on estradiol, which is not something the evidence explains well but is not nothing either.
There is also an ethical dimension that some women care about and others do not, relating to how the drug is sourced from horses. That is a personal call rather than a medical one, but it is a real reason some people ask to switch.
If you are on Premarin and would prefer transdermal estradiol, the conversation is straightforward. The reasons that carry weight are clinical rather than preferential: a family history of clots, migraine with aura, raised blood pressure, or cost. A personal clot history is a separate conversation, not a route preference, because the label lists a prior clot, stroke or heart attack as a contraindication for systemic estrogen by any route. Ask specifically whether a patch or gel would suit you better, and what the equivalent dose would be.
If you are being started on hormone therapy now and Premarin is offered without discussion, it is fair to ask why an oral conjugated estrogen rather than a transdermal estradiol, given the guidance. Our list of questions is built for exactly this kind of appointment, and our comparison of HRT types lays the options out side by side.
Estradiol with a transdermal option is where most menopause specialists now start, mainly because the route lowers clot risk and the generic is inexpensive. Premarin remains a legitimate treatment with a long record, and being on it is not a reason to panic. If you are choosing today, the ability to take estrogen through the skin is the argument that decides it for most people.
